If you have seen any kind of suspicious activity in Pittsburgh, please complete and submit the following form to the Police Bureau.  Providing your name and contact information is optional; however, please be sure to fill out all other fields in as detailed a manner as possible.  That way we can follow up on the manner as quickly and effectively as possible. 

If you prefer, print a copy of the form and complete it, then send it in to a zone police station.
